Care management over site can be provided in a home, hospital, and assisted living setting. Recommendations for home care agencies or registries are providers we have worked closely and formed positive relationships with. We base our recommendations to these providers on client's care needs and personalities. We provide drop in visits at varying times of day to evaluate if our plan is working and if the home care agency is a good fit for meeting our goals. Physician input and medical oversight is important to the clients overall well being and stabilization.
