Moving between facilities or from home to a facility can be challenging. A care manager can coordinate and or assist families in all components of relocation. This can include assistance with facility or housing recommendations, coordination of movers, and assistance with client transport when appropriate. A care manager can discuss the move with the old and new facility ensuring that the clients special needs, concerns, or requests are communicated to the new staff and followed through.
